New Patients
New Patient Forms
We have made our new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home. Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.
Your First Visit
At your first visit, arrive about 15 minutes early to complete registration forms, or download the forms on this page and complete them at home. Once registration is complete, you’ll meet with our hygienist first, and then with Marla, Dr. Danner’s assistant, and Dr. Danner. We’ll listen carefully to your concerns and answer all of your questions. You can expect a thorough oral examination, including:
- Health history review
- DIAGNOdent laser cavity detection
- Oral cancer assessment
- Necessary digital X-rays
- Intraoral photos
- Head, neck, occlusion examination
- Hygiene consult and cleaning (in absence of gum disease)
- 3D Scan Imaging
Dr. Danner will review this information, focusing on your immediate and long term goals. Then he’ll enter a thorough discussion with you regarding the best treatment plan to meet your unique oral health goals.
We believe, the more you know about treatment options, the more prepared you’ll be to make informed decisions. Dr. Danner will review your X-rays and intraoral images, and he may show you brief videos to explain potential solutions. As your personal, professional dental consultant, Dr. Danner will help you determine the best path to improve and maintain your sensational smile.
